TECHNICAL INVESTIGATION OF ''EXPLODING'' FLARE FATALITY.

Abstract

While performing routine candlepower measurements of a pyrotechnic flare, a project engineer was fatally injured. The injury occurred from a door blown open by unexpected explosion of the flare. This report covers the details of the incident and the technical investigation performed to determine the cause. Conclusions are reached pertaining to the cause for the flare reacting in an unpredicted manner and previous inadequacies of the design of photometric tunnels. (Author)
