CERTAIN ASPECTS OF THE USE OF CORTICOSTEROIDS IN HEART SURGERY,

Abstract

During recent years, adrenal cortex preparations and glycocorticoids in particular have come into extensive use in general and thoracic surgery. This is due to the fact that the glycocorticoids (cortisone, hydrocortisone, prednisolone) and their synthetic analogs exhibit a broad pharmacological and therapeutic spectrum of activity. They exert a pronounced influence on the cardiovascular system and on all forms of metabolism and are powerful antiphlogistic, antiexudative and antiallergic agents. In this article only their application in cases of acute vascular insufficiency and cases of exudative pleurisy and pericarditis is considered. The purposes of their use and physiological effects are described for typical cases in each category. Adverse side effects, noted in some cases, are described. The chief value of corticosteroids lies in their effective moderation of traumatic pleurisy and pericarditis and activation of the rheumatic process and by helping to maintain arterial pressure.
