POTENTIAL CAUSES AND CONSEQUENCES OF SPACE-FLIGHT EMERGENCIES,

Abstract

On the basis of a survey of potential causes of emergencies in space, the document considers that these may be ranked, in order of importance, as (1) equipment failures, (2) personnel errors, and (3) environmental hazards. A fourth potential cause, personnel breakdowns, appears to be of negligible significance relative to the other causes. (Author)
