PAROTID FLUID CORTISOL AND CORTISONE.

Abstract

Concentrations of parotid fluid cortisol and cortisone, and of plasma dialyzable cortisol, were measured in normal men before and 2 hours after treatment with 40 units of ACTH, and also before and after 10 days of receiving 5 mg. of diethylstilbestrol daily. Total plasma cortisol rose from a mean of 8.5 to 17.7 micrograms/100 ml. after ACTH treatment, and to 39.4 micrograms/100 ml. after administration of the estrogen. However, parotid fluid cortisol plus cortisone rose from 1.1 to 2.7 micrograms/100 ml. after ACTH treatment, and to 2.2 micrograms/100 ml. after administration of estrogen. This rise resembled that of the plasma dialyzable cortisol (control 0.5 micrograms/100 ml.; ACTH 1.8 micrograms; estrogen 1.2 micrograms) rather than the increase in total plasma cortisol (which was over twice as high after administration of estrogen as of ACTH). Thus parotid fluid corticosteroids seem to be a reliable measure of nonprotein-bound corticosteroid--the cortisol available to the cell. In addition to serving as a filter for plasma dialyzable cortisol, the parotid gland appears to reabsorb a portion of this corticosteroid in proportion to the amount in the 'filtrate.' The total amount of cortisol plus cortisone excreted is approximately constant and is independent of the flow rate of parotid fluid. Cortisone exceeds cortisol in parotid fluid in the basal state, but after ACTH treatment the situation is reversed. (Author)
