AN AIRCRAFT ACCIDENT REPORT FORM AND MANUAL FOR USE BY FLIGHT SURGEONS,

Abstract

The object of the study was to develop a form and a manual to be used by Navy Flight Surgeons in preparing diagnostic reports of aircraft accidents. A review was made of the literature on aircraft accidents and report forms. The problem of reporting accidents was discussed with a sample of Navy Flight Surgeons and pilots, safety officers and members of safety boards, and with members of various Government agencies concerned with flight safety. A preliminary version of the accident report form was evaluated by Flight Surgeons and modified in line with their suggestions to become the form shown in the Appendix. It is anticipated that follow-up studies will be conducted to test the reliability of the form under service conditions. (Author)
