Alterations in Tissue Metabolism (the Lung) with Injury and Shock.
Abstract
The purpose of work carried out under this contract was to investigate cellular injury with shock, particularly as it might relate to organ failure and death in combat casualties. Particular emphasis was placed upon the lung because of the problem of ventilatory or respiratory failure after severe injury which has been encountered in Vietnam. Details of the studies of cells and cell metabolism in shock centered initially about three tissues in experimental animals including the lung, the brain and the liver with an experimental hemorrhagic shock model with studies of mitochondrial metabolism, microsomal enzymes and cation alterations within cells. Progressive alterations in cellular and subcellular function were found particularly in the liver and were produced in vivo by hemorrhagic shock. There was limitation of the metabolic capability of mitochondria which was most marked in the more complex portions of the Krebs cycle. At the same time the metabolic capabilities were decreasing, there were marked alterations in the enzyme system thought to be involved in sodium and potassium transport and this seemed to be related to the alterations in cation content of mitochondria with a decrease in potassium and an increase in sodium and the lability of magnesium. Shock may produce an initial change in membrane permeability both of the cell and mitochondria. During this period of activity such changes were not observed in the lungs of the experimental animals which has raised the question as to whether or not hemorrhagic shock in itself does, indeed, alter pulmonary tissue.
