ACCIDENT INVESTIGATION REPORT MISSILE 5A STATIC TEST STAND 1-1.
Abstract
Tests were conducted in accordance with the requirements of Test Directive. This was the fourth full duration static firing to be conducted with the 5-A missile. Test objectives and hardware configuration were similar to those for the three previous full duration firings. A primary objective was the investigation of thrust section environment with the heat radiation shield and anti-fire boots installed, but without the GG exhaust extension. Test operations proceeded somewhat slowly because of incomplete airborne pneumatic system instrumentation, with 'vernier start depress' occurring. Engine performance was satisfactory, and cutoff was initiated by the flight programmer after approximately 131 seconds of vernier and 128 seconds of booster operation. A small fire was noted at cutoff in the vicinity of the B2 thrust chamber, but this was quickly extinguished. During post test inspection and securing operations, the missile LO2 tank was found to be wrinkled and the missile tank intermediate bulkhead ruptured. (Author)
