Abstract
This report of missile 60-5502 incident at Forbes AFB is presented to document the findings of a Special Investigating Team. Included in the report is a detailed description of the efforts of the Team in determining the cause(s) of the incident. The details in this report include: extent of damage in the thrust section of the missile; all tests accomplished by the team; compilation of all data observed and recorded in testing; and the conclusions and recommendations of the team.
