Maintenance of health care providers clinical proficiency: Transdisciplinary analysis, modeling and intervention
Abstract
This 3-year partnership of the Nemours Foundation and Lockheed Martin, will: 1.) Evaluate whether physicians frequency of exposure to targeted clinical problems is associated with decay in physicians cognitive clinical skills; 2.) Develop and test multivariate explanatory models of decay in physicians clinical decision making; and 3.) Develop/test educational, behavioral and electronic interventions to minimize effects of skill decay on health care quality. During Year 2, we have completed selection of targeted clinical problems, identified 9 clinical problems for further study, conducted formative interviews of physician subject matter experts, collected massive amounts of electronic health record data and data on worker and workplace characteristics, demonstrated skill decay pertaining to certain elements of care pathways, and continue to test and refine initial multivariate models that were developed. We have initiated an intervention trial targeting physicians' influenza immunization practices and are soon to begin trials targeting gastroesophageal reflux disease, supracondylar fracture and concussion.
