Building Diabetes Champions in Primary Care: The Air Force Experience

Abstract

The US Air Force Diabetes Center of Excellence designed the Diabetes Champion Course (DCC), a semi-annual, 3-day course to train primary care teams to become local champions in diabetes standards of care. Didactics and hands-on training are focused on current standards, the latest technologies, and patient flow in a team-based setting. Each team is tasked to identify local deficits and make a Plan of Action (POA) for implementation.
