Successful Carboplatin Desensitization Without Steroids in an Adolescent Patient with Low Grade Glioma

Abstract

Low grade gliomas (LGG) are common among pediatric brain tumors. Standardof-care treatment for LGG includes carboplatin. Carboplatin causes IgE-mediated and/or Type IV hypersensitivity reactions in 1% of administrations. Successful carboplatin desensitization has been well described in treatment of adult gynecological tumors and commonly utilizes pretreatment with systemic steroids. Limited success has been reported to carboplatin desensitization in children with LGG and most have utilized systemic steroids as pretreatment or to treat hypersensitivity reactions. We present a successful modified steroid-free protocol for a pediatric patient with LGG and a history of significant steroid-induced psychosis; this protocol has been previously reported only in adults with gynecological tumors.
