A SOF Damage Control Resuscitation Cocktail
Abstract
Objective: The goal of this project is to develop a new damage control resuscitation (DCR) cocktail made of fibrinogen, Hextend, Vasopressin, and tranexamic acid for use by SOFs that is capable of improving survival from polytrauma in austere settings. These components were tested in a swine polytrauma model of hemorrhagic shock with traumatic brain injury; free internal bleeding from an aortic tear, and femur fracture treated using tactical combat casualty care (TCCC) over 6 hours. Results: Fibrinogen concentrate decreased internal blood loss and enabled 3 animals to survive to 6 hours while also enhancing brain and vital organ blood flow. There was no effect of TXA in this model. In conclusion, our cocktail consisting of Hextend, Vasopressin, and fibrinogen concentrate decreased blood loss and enabled survival to 6 hours in an otherwise lethal swine polytrauma model.
