Improving Outcomes Using Myelotomy with Intramedullary Hemorrhagic Necrosis Removal in Porcine Model of Acute Thoracic Cord Injury
Abstract
During the past year, we published a manuscript establishing the treadmill-based kinematics in the healthy Yucatan pig. Thispaper showed for first time that Yucatan minipigs use a lateral footfall sequence locomotion pattern. At all speeds tested, strideand stance durations decreased with increasing speed whereas the swing durations were not significantly affected. Thevariability of stepping was higher at the lower speeds, and the animals reached a greater consistency between 2.03.0 km/h.Based on these studies, a speed of 2.5 km/h (0.7 m/s) is recommended for future preclinical studies. We also made significantnew discoveries analyzing MRI data. For first time, we have demonstrated that T2-weighted imaging does not providereproducible data for determining spinal cord injury near and remote to the focus of injury in the spinal cord in this model. ourearly data demonstrate that MRI myelography, a technique not routinely used in clinical practice is significantly more sensitiveand specific than currently used standard T2-weighted imaging and can detect cord injury both adjacent to and remote to thesite of injury. Preliminary MRI myelography analysis demonstrates an increase in cord permeability occurring remotely fromthe cord injury site demonstrated by a positive normalized contrast to noise ratio comparing the normalized signal within bothgray and white matter structures of the cord.
