Group Medical Visits for Chronic Conditions

Abstract

Chronic conditions are the leading cause of death and the largest financial burden in the United States. The Military Health System faces challenges in providing quality care for chronic conditions. Clinical Question or Purpose: Clinical Question: for primary care patients with chronic conditions, do group medical visits compared to standard individual visits, improve or match health and wellness outcomes? Purpose: Introduce healthcare providers to the Group Medical Visit (GMV) model for the treatment of chronic conditions in primary care. Project Design: The Iowa Model Revised: Evidence-Based Practice guided the project design. The project had two parts, Introduction to GMVs for Chronic Conditions and GMV Working Group. The goals of the project were to increase knowledge about GMVs, assess perceptions about GMVs, and to inspire the clinic to implement GMVs. The setting was Winder Family Medical Clinic (FMC), Madigan Army Medical Center, Tacoma, WA. Analysis of the Results: The literature demonstrated that care provided by GMVs outperformed or matched standard individual visits in health metrics for multiple chronic conditions. GMVs have high levels of patient and provider satisfaction and encourage patient empowerment, allow for shared decision-making, and facilitate peer support. For part one of the project, Introduction to GMVs for Chronic Conditions, 53 participants attended, and 20 (46 ) answered affirmatively to wanting to start GMVs if there were resources. For part two, GMV Working Group, 44 participants attended, of which 20 participants (46 ) answered affirmatively as interested in seeking more information on GMVs. Twenty-one participants (49 ) plan to be part of a GMV team, including two provider champions. Organizational Impact/Implications for Practice: Two stakeholders, who are providers at Winder FMC, expressed a strong desire to champion GMVs and are establishing a team.
