Systemic Lupus: Improving the Rationale for Treatment Choices in a Heterogenous Disease
Abstract
The aim of this project was to compare impact of methotrexate to that of other common lupus immunomodulatory medications(azathioprine, mycophenolate and hydroxychloroquine) on interferon-inducible genes, and as warranted, other patientimmunophenotypic clusters. 564 paxgene and 543 plasma tubes have been selected at relevant timepoints from 154 patientsto provide samples from active/inactive disease on no immune suppressants, methotrexate, and other common treatments ascontrols. We completed the RNA expression array of 360 genes. Major findings during this project include confirmation ofmethotrexate impact on patients with a high interferon signature, and dampening effects on multiple pathways and on a panelof IRGs that we detected. Interference with observable methotrexate effects were found with use of concomitanthydroxychloroquine or corticosteroids. Taken together these observations confirm the feasibility of using immunologic markersto improve selection of treatments and dosing optimization for individuals with a heterogenous disease such as SLE.
