The Impact of Cardiovascular Disease and Intervention through Modifiable Risk Factors in the Primary Care Behavioral Health (PCBH) Model

Abstract

Cardiovascular disease (CVD) remains the leading cause of mortality in the United States after nearly a century. Although developments in interventions for CVD have improved the survival rate of major adverse cardiac events, the pervasiveness of poor cardiovascular health remains a growing public health burden with deleterious impacts at societal, organizational, and individual levels.
