Prehospital Kcentra for Trauma Patients with Hemorrhagic Shock
Abstract
Exsanguination remains the leading cause of preventable death after trauma and a large number of these death occur in the out of hospital setting. Field use of plasma has been shown to result in improved survival in civilian trauma patients compared to crystalloid but plasma (PAMPER Trial - Sperry et al) is frequently unavailable in out of hospital settings and it may have to be stored frozen and then thawed making it logistically prohibitive for routine use in the field. 4-Factor Prothrombin Complex Concentrates (4FPCCs) are readily available and provide many of the same beneficial proteins that are present in plasma and could serve as logistically feasible substitutes in the out of hospital settings. Kcentra is a 4FPCC product. A single bolus of Kcentra carries logistic and therapeutic promise because the treatment may limit blood loss, can be carried on the battlefield, administered rapidly and easily used in the prolonged field care of wounded warfighters. Therefore, we hypothesize that Kcentra given in the field to patients with traumatic hemorrhagic shock will result in improved survival, decreased end organ failure and no increase in thromboembolic complications.
