An Investigation of Safety Hazards and Reliability Problems in Aerial Refueling Stores.

Abstract

An investigation was conducted to identify sources of safety hazards and reliability problems in model D-704 aerial refueling stores. A failure modes and effects analysis was performed on the D-704. Mishap data, safety UR (unsatisfactory report) data, and 3-M (maintenance and material management) data were analyzed to identify historical problems. Comparisons of model 31-300 failure statistics to model D-704 failure statistics is included.
