Abstract

This report documents the investigation of the crossdeck pendant failure aboard USS JOHN F. KENNEDY (CV 67) in March 1978, which caused the loss of an F-14A. Included are the results of a survey to determine the scope of the deck pendant/catapult slot interaction problem, testing to confirm the failure sequence, and the evaluation of slot filler devices (buttons) to resolve the problem.
