A Medical Monitoring Program for the Marine Hazardous Chemical Worker. Volume 1

Abstract

This document is Volume I of a three-volume report which describes the development of a medical monitoring program for marine personnel who may be potentially exposed to hazardous chemicals during their work. The purpose of the program is to screen for adverse health effects of occupational exposures to noise and toxic chemical substances. A basic medical protocol for periodic screening examinations and a target organ protocol for examinations performed in response to acute overexposure to specific toxic substances are described. A recommended schedule for medical examinations is specified and guidelines for follow ups of abnormal laboratory results are provided. Auxiliary systems to support implementation and application for the medical monitoring system include: 1) A hazardous substance data system for nearly 700 marine hazardous substances, 2) A set of biochemical and medical data sheets for 179 of the more toxic marine hazardous substances, 3) Guidelines for biological monitoring of exposures to 26 marine hazardous substances for which methods are available, 4) Two methods for documentation of potential exposures to marine hazardous chemicals: one which is self-documented, and a second, which employs an indirect method based on cargo manifests and personnel records 5) Guidelines for an industrial hygiene program to complement medical monitoring, and 6) A data management system to provide a record of data and to provide a means of rapid access to data files for specific information and for data summaries.
