Root Cause Analysis Methodology

Abstract

A Root Cause Analysis is performed to identify the real source (i.e., root cause) of a problem. The process includes a sequential series of steps that will both determine the root causes as well as document the basis for this determination. There are many different technique in use to achieve this same end and are referred to as Failure Analyses, Problem Investigations, etc. A particular Root Cause Analysis Methodology that is employed throughout AMCCOM will be presented. The purpose of this report is to present a description of the specific steps involved in this methodology including various procedures that aid in achieving thoroughness and objectivity.
