Gateway to Care Implementation and Business Plan: How to Get There from Here.

Abstract

The spiraling cost of health care within the Department of Defense (DoD) forced an evaluation of the effectiveness of military medicine. In 1988, DoD instituted two demonstration projects to test the feasibility of alternative delivery systems within the military. CHAMPUS Reform Initiative (CRI) in California and Hawaii, and 5 Catchment Area Management (CAM) projects (2 Air Force, 2 Army, and 1 Navy location). In January of 1992 Dr. Enrique Mendez, the Assistant Secretary Defense (Health Affairs), introduced policy directing the uniformed services to begin implementation of an admixture of CRI and CAM entitled Coordinated Care. The proposed Coordinated Care Program (CCP) was poorly received in Congress and not well understood across Health Services Command (HSC). Although the concept of CCP was adopted, the functional expertise to implement such a program was sorely lacking.
