A Fiscal Intermediary Based on Needs Assessment to Support a Managed Care Approach to Outpatient Child and Adolescent Psychiatry Care at BAMC.

Abstract

Child and adolescent psychiatry services are virtually unavailable in house at the treatment facilities of the Joint Military Medical Command (JMMC), San Antonio, Texas. The cost for this service under the Civilian Health and Medical Program of the Uniformed Services (CHAMPUS is exorbitant and growing steadily. The JMMC has begun a managed care project to bring the treatment in house with outpatient care at Brooke Army Medical Center (BAMC). Although plans have been made to implement the services in 1991, a detailed catchment area needs assessment has not been conducted. The author used a CHAMPUS billing database known as the Quick Response Detail Files (QRDF) which was collected by the Fiscal Intermediary and archived to tape by CHAMPUS. The QRDF files were formatted and uploaded into the Statistical Application System (SAS) software to facilitate manipulation and analysis. The data was used to determine the patient needs for the catchment area and to attempt to construct a patient profile for child and adolescent /services.
