Definition of Roles and Responsibilities of Health Care Team Members in a Population-Based Model of Primary Health Care Delivery

Abstract

Group Health Cooperative of Puget Sound is currently designed to provide episodic care to patients in the primary care setting. In order to meet the preventive, self-limiting, episodic, and chronic care of its enrollees, the Cooperative has decided to take a population-based approach to delivering care. The challenge is to integrate the population-based approach into practices of Group Health's primary care provider teams. Reengineering patient care delivery models requires fundamental changes in the jobs and work lives of care providers. The population-based approach to medicine and the funiamental changes in roles and responsibilities that are needed to support a population-based approach to health care delivery are described.
