Military Aircraft Accident Investigation and Reporting

Abstract

This evaluation was initiated in response to a request from the Deputy Under Secretary of Defense (Installations and Environment), to conduct a process review of the Military Service's implementation of the DoD policy as stated in DoD Instruction 6055.7, "Mishap Investigation, Reporting, and Recordkeeping". April 10, 1989. The Deputy Under Secretary of Defense (Installations and Environment) asserted that improving aviation safety was a major objective of DoD. Military Department aviation safety programs focused on the reduction and prevention of aviation mishaps; however, aviation mishaps continued to injure people and caused property damage and losses. During FY 2000, the Services experienced a total of 57 class ilAls aircraft mishaps, which resulted in 58 deaths and 44 aircraft destroyed. A class "A" mishap occurred when the reportable damage was $1 million or more; there was total destruction of the aircraft; or an injury resulted in a fatality or permanent total disability.
