Liver Transplantation in CHAMPUS: A Total Patient Treatment Episode Detailed Analysis.

Abstract

This report presents a detailed analysis of liver transplantation within the Department of Defense Civilian Health and Medical Program for the Uniformed Services (CHAMPUS). The source of data was the Tri-Service CHAMPUS Statistical Database Project (TCSDP). Patients which underwent liver transplantation procedure paid during Fiscal Year (FY) 1990 or FY 1991, were included. A number of software techniques were employed to develop a liver transplantation total patient treatment episode. There was a grand total of 167 hospitalization episodes where a liver transplantation procedure was paid during either FY 1990 or FY 1991. The total billed charges were $45.2 million (M). Total amount paid was $39.2M and government liability was $33.4M. Due to the time lag between when care is rendered and paid, hospital discharges prior to FY 1990 and after FY 1991 were eliminated prior to detailed analysis. There were 100 episodes where the hospitalization ended during FY 1990-1991. The average age was 32.96 years Recipients were split evenly with respect to branch of service and gender, and tended to be dependents of the sponsor. Four episodes had evidence of either liver re-transplantation or significant complications to warrant the extended reimbursement of the transplantation procedure. In seven episodes there was a record of a previous hospitalization where a liver transplantation on that patient was reported. of the 100 episodes, 22 had evidence of liver transplantation rejection, and 11 patients expired prior to liver transplantation hospitalization discharge. The transplantation hospitalization had a mean length of stay (LOS) of 46.84 days and a mean total reimbursement of $231,345. Overall episode LOS was 57.87
