Zambia Defense Force (ZDF) Care and Treatment
Abstract
In response to Department of Defense (DoD) HIV/AIDS Prevention Program (DHAPP) BAA-15-002, FHI 360 will continue collaboration with the Zambia Defense Force (ZDF) and Ministry of Health (MoH) to reduce new HIV infections among military personnel, their families, and the surrounding communities. Through the ZDF-Care and Treatment (C&T) project, FHI 360 will support the Defense Force Medical Services (DFMS) to aggressively scale up and improve the availability and quality of the following services at target health facilities: adult HIV/AIDS clinical care, pediatric HIV/AIDS clinical care and support, TB/HIV integration and co-infection management, and pediatric and adult antiretroviral therapy (ART). The Goal of the ZDF-C&T is to support the ZDF in scaling up care, treatment, and support for adults and children living with HIV and TB. The purpose of the ZDF-C&T project will be to control the epidemic among Zambia’s military by moving closer to achieving the “90-90-90” aims: 90% of military personnel, their families, and communities who are people living with HIV (PLHIV) know their status; 90% of those diagnosed are on ART; and 90% of those on ART are virally suppressed by 2020. The project has 3 Objectives: (1) To strengthen and scale up HIV/AIDS care and treatment services in adults and children in at least 54 facilities by 2020; (2) To strengthen and scale up TB diagnosis, care, and treatment in adults and children in at least 54 facilities by 2020; and (3) To increase the technical and programmatic capacity of the DFMS to conduct and manage HIV/AIDS programs in a sustainable manner by September 2020. Strategy: FHI 360 has worked with DHAPP, the ZDF and MoH over the past four years of the ZDF Prevention, Care and Treatment (ZDFPCT) project and will build on the successes and lessons learned from ZDFPCT. FHI 360 will support access to quality HIV/AIDS services for PLHIV by incorporating a unique mix of international evidence-based best practices, on-the-ground experience with HIV C&T, and in-depth knowledge of the local context. To support 241,302 beneficiaries throughout 7 high burden provinces, ZDF-C&T will reach 54 facilities by continuing the effective activities implemented under ZDFPCT and scaling up new strategies implemented during Y1 of ZDF-C&T. In Lusaka, Central, and Copperbelt, FHI360 will employ a “saturation model” to rapidly scale up HIV C&T services to reach 80% saturation of ART coverage among PLHIV who are military personnel, their families, and the surrounding communities. In Muchinga, Northern, Northwestern, Luapula, Southern, Eastern and Western, the project will use a targeted approach to provide interventions at HIV high-maintenance sites with high patient volume. In addition, ZDF-C&T will undertake a revised capacity development and transition approach to address the lack of transition of facilities from ZDFPCT to ZDF. The transition will be driven by the PEPFAR 3.0 country strategy for transitioning sites and will be based on the rigorous use of QA/QI tools which encompass technical strategy, data management, and human resources. A fundamental element of the ZDF-C&T approach will be assisting the ZDF, MoH, PLHIV groups, and target facilities to rapidly and efficiently increase availability and uptake of HIV services across the prevention-care-treatment continuum. ZDF-C&T will use the HIV cascade framework, a powerful advocacy, planning, and monitoring tool that can be easily understood and utilized by all actors in the HIV response.
