HIV/AIDS Care and Treatment PEPFAR Priority Regions of Cameroon

Abstract

Since 2000, Metabiota, Inc. has provided the Cameroon Defense Forces (CDF) technical assistance and direct service delivery for their HIV program. In FY19, Metabiota proposes to optimize HIV-yield, linkage and retention services for HIV/AIDS Care and Treatment in Cameroonian Military Health facilities, as well as provide community-based priority prevention activities in-and-around military barracks. In addition, optimal clinical and biological care services (ART, CD4, PCR, VL), will be provided to PLHIV enrolled in the military health facilities in the four PEPFAR priority regions of Cameroon. Specifically, the proposed activities will be implemented:  Increase HIV-yield through testing of high-risk individuals through various testing modalities (emergency, inpatient, VCT, PITC, pediatric) and systematic testing of most-at–risk individuals through index case testing, TB and PMTCT testing modalities, quality laboratory procedures, and linkage of HIV+ individuals into treatment, in the military health facilities. o Average yield of 7% for targeted among high-risk individuals o Average yield of 20% for index case testing; o Average linkage of at least 90% of identified HIV positive individuals into ART.  Improve the quality of care and treatment, adherence to care, retention to treatment, as well as viral load coverage and suppression for all PLHIV (infants, children, teenagers, pregnant women and other adults) in the military health facilities. o At least 90% of PLHIV on ART are maintained in care and treatment; o At least 30% of eligible PLHIV are provided with VL tests – preference will be given to children, teenagers, active duty militaries and other adults who cannot afford testing; o At least 90% of PLHIV with VL results will be virally suppressed.  Provide appropriate TA to transition MTCT testing to the CDF in all military health facilities, and ensure that HIV+ pregnant women are initiated and retained on ART to reduce risk of mother-to child transmission during pregnancy. o HIV testing services for pregnant women in all PEPFAR-supported military health facilities are transitioned to CDF/MoH; o At least 90% of HIV+ pregnant women on ART are maintained in care and treatment; o At least 60% of HIV+ pregnant women are provided with VL tests; o At least 90% of HIV+ pregnant women with VL results are virally suppressed; o At least 90% of HIV-exposed infants are provided EID and PEP services.  Provide high quality prevention and tailored messaging for young military (ADM) and adolescent girls and young women (AGYW) living within and around military barracks, in an effort to prevent new infections, empower both young military and AGYW with positive life skills, and promote medical seeking behavior in relation to HIV or other health related issues. o Group-specific sensitization materials will be produced and distributed accordingly o Priority prevention education sessions are organized for ADM and AGYW; o Targeted testing will be provided to high risk individuals living within and around military barracks (targeted community / mobile testing); o At least 90% of identified positives will be linked to ART; o Advocacy meetings will be held with the military command; o Support groups will be created and maintained in all sites.  Support the use of disaggregated data by military versus other beneficiary institutions, and the reporting of site-level data to DHAPP HQ and the CDF leadership of the clinical, community and other programmatic data for decision-making. o Consolidated, disaggregated data collected and made available for project reporting; o Data made available and used for strategic decision-making and project management.
