Cross Cutting in Military Settings
Abstract
Jhpiego’s goal is to support the Botswana Defence Force (BDF) and DOD/DHAPP to reduce the number of new HIV infections and other sexually transmitted infections among members of the BDF, their families and the civilian communities served by the BDF. The scale up of voluntary medical male circumcision (VMMC), HIV testing services (HTS), clinical laboratory support and viral load testing, will support achievement of the 90-90-90 goals for HIV epidemic control in the military. By the end of Year Three, the program will achieve the following objectives: Objective 1 (VMMC): Support the BDF to deliver and scale up a comprehensive VMMC package in alignment with PEPFAR guidance, World Health Organization recommendations and national guidelines, including: demand creation; education and sexual risk behavior reduction (including condom provision); HIV counseling and testing; screening and treatment of sexually transmitted infections; surgical circumcision; post-operative care and continual counseling and education on abstinence during the six-week healing period; as well as correct and consistent use of condoms indefinitely thereafter. By the end of Year One, the program will conduct 600 circumcisions within the military and 4,700 in the civilian population. Objective 2 (HIV/AIDS Care and Treatment): Scale up viral load testing coverage by supporting and scaling up viral load testing at four fixed sites. By the end of Year One, the program will ensure that 4,000 patients on antiretroviral therapy are virally suppressed. Objective 3 (HTS): Provide HTS to military bases and facilities, including the implementation of self-testing, conducting proficiency testing, monitoring HIV testing yields, ensuring linkages to services, and providing quality improvement and quality assurance. By the end of Year One, the program will provide HTS to 1,800 military and 4,200 civilians; identify at least 34 military and 136 civilian new HIV-positive clients and link at least 90% to care; and distribute at least 1,000 HIV self-test kits to military personnel. Objective 4 (Clinical Laboratory Support): Ensure high-quality, uninterrupted HIV clinical laboratory services that support HIV prevention, care and treatment. Jhpiego will provide support for inventory control, forecasting and procurement; assist with equipment maintenance; ensure laboratory quality and continuous quality improvement; and support collaboration and linkages with other labs. Objective 5 (HIV Monitoring Systems): Improve tracking of patient-level data and use of data for clinical and programmatic decision-making. Jhpiego will support and mentor BDF staff on data entry, ensure confidentiality and security of data, provide quality assurance/quality improvement and provide support for data entry, cleaning and reporting. Overall, these five objectives will be accomplished through coordinated strategies that build BDF capacity at all levels. Jhpiego will consistently engage BDF in program planning and implementation, building capacity through joint planning and training sessions and built-in sustainability plans for program activities. Jhpiego has actively engaged with the BDF during the development of this proposal and BDF is supportive of the proposed program.
