DHAPP-MDF Partnership for Sustainable HIV Epidemic Control
Abstract
The proposed project will strengthen the continuum of HIV prevention, care and treatment for MDF personnel, their families and civilian communities served by MDF, contributing to attainment of the 90-90-90 targets toward sustainable HIV epidemic control while building MDF’s capacity to sustain this work beyond end of project. Jhpiego will draw on its 20 years of experience in Malawi as a trusted MDF, DOD, and Centers for Disease Control and Prevention partner supporting high-quality service delivery, building capacity and supporting comprehensive HIV services. Building on strong partnerships in Malawi and strong regional experience, including in Mozambique, where Jhpiego implements the DOD comprehensive HIV services project that retains 95% of military clients in treatment after 12 months, Jhpiego will: provide standardized, evidence-based interventions to promote adoption of HIV prevention behaviors and service uptake for all military and other uniformed services as well as priority populations (Objective 1); offer a full package of voluntary medical male circumcision (VMMC) services and implement a targeted VMMC strategy to reach specific subpopulations of men in line with the US President’s Emergency Plan for AIDS Relief (PEPFAR) guidance, World Health Organization recommendations and national guidelines (Objective 2); implement high-quality, targeted HIV testing services (HTS) to achieve 90% of all MDF people living with HIV (PLHIV) knowing their HIV status and linking at least 90% of those testing HIV-positive to antiretroviral therapy (ART) (Objective 3); provide HIV treatment services to ensure that 90% of diagnosed PLHIV in the project catchment area are on sustained ART (Objective 4); scale up viral load (VL) testing coverage to achieve 90% viral suppression among PLHIV on ART (Objective 5); expand the functionality, quality and use of the existing military health information system (HIS) and ensure the transition to utilization of the HIS as the primary data source for PEPFAR and Ministry of Health reporting to drive quality services (Objective 6); and ensure high-quality service provision that meets national/international standards, is guided by principles of using data for decision-making, and reflects appropriate partnerships and capacity-building to transition the project to MDF at project end (Objective 7). The project will address the elevated risk of HIV infection among the military, challenges of working with a mobile population, gaps in human resources and capacity, low HIV service uptake among men and adolescent girls and young women, and inadequate data quality and use for decision-making. With MDF and DOD/DHAPP, Jhpiego will support HIV services in 13 high-priority MDF facilities (11 of which provide ART and two of which are Determined, Resilient, Empowered, AIDS-free, Mentored and Safe [DREAMS] sites) for: aggressive strengthening of prevention services, including DREAMS and VMMC; targeted HTS, including provider-initiated testing and counseling in high-yield clinics, index testing, and HIV self-testing; active linkage using expert clients to facilitate same-day ART; treatment, including transition to tenofovir/lamivudine/dolutegravir and differentiated care models; adherence support to ensure retention and viral suppression; strengthened laboratory capacity and VL testing; and installation of electronic medical records and expansion of the HIS. Jhpiego will support MDF with collection, analysis, reporting and effective use of high-quality data to improve clinical and facility-based decision-making. With MDF and DOD/DHAPP, Jhpiego will leverage the military’s organizational structure to implement activities in line with MDF and DOD/DHAPP priorities. Transition of quality services to MDF is a priority and will be fostered via ongoing capacity-building and support for improved project planning, oversight and coordination.
