Wiggers-Bernard Conference Initiative for Pre-clinical Modeling in Sepsis Research
Abstract
Context and Background: Pre-clinical sepsis research has not been subjected to any organizedattempt at introducing best practices and/or management guidelines. In May 2017 in Vienna, ourinstitute organized a Wiggers-Bernard Conference (W-BC) on ~Pre-clinical Modeling in Sepsis:Exchanging Opinions and Forming Recommendations.~ W-BC created 29Recommendation/Consideration consensus points within the so-called ~Minimum Quality Thresholdin Pre-Clinical Sepsis Studies~ (MQTiPSS) that were disseminated as an Executive Summary(simultaneous publication in 3 scientific journals; PMID: 30106875; 30112605; 30105433) and threefull-size papers (in journal SHOCK, 2019 January issue: https://journals.lww.com/shockjournal/Pages/currenttoc.aspx).Another pervasive flaw is the low reliability of pre-clinical sepsis findings in predictingoutcomes/effects in septic patients. The reasons for the poor prediction include lax control of animalexperiments, poor compliance with existing quality protocols (e.g. ARRIVE), large disparity in appliedprotocols and the general philosophy of single-laboratory-type of studies that are difficult to crosscompare.These (and other) problems vastly contribute to the ~reproducibility crisis~ on the animalresearchlevel (PMID 25552691).Plan and Objectives: To address the reproducibility deficit, LBI Trauma organizes another W-BCtitled: ~Pre-clinical Modeling in Sepsis: multicenter randomized control sepsis trial~, an initiativecentered on developing a multi-center format for pre-clinical sepsis research. The W-BC will takeplace in October 12, 2019 in Chania, Greece. The W-BC is by-invitation-only event that will gather aselection of top international experts (28 participants + 2 external experts) in sepsis modeling.The key objective of the 2019 W-BC is: feasibility of designing and executing a pre-clinical multicentertrial (pRCT) in sepsis models as a tool for enhancing experimental reproducibility. Theconference will be proceed by a preparatory process during which the participants will contemplatethe most feasible design option (via SurveyMonkey and iDelfi questionnaires) to frame an entry studyprotocol for pRCT. This entry protocol will be subsequently shaped into the defined blueprints at themeeting in Crete. The conference will also discuss the impact of the published MQTiPSS, itsdissemination and its role in promoting the pRCT in animal sepsis research.
