Identifying and Reducing Disparities in Symptom Burden Among African American Prostate Cancer Survivors
Abstract
We propose a project to improve quality of life for prostate cancer survivors by identifying disparities in symptom burden as well as risk factors for symptom burden disparities among African American prostate cancer survivors. Few studies have collected patient-reported outcomes from African American prostate cancer survivors, but these studies have generally found worse outcomes in this population than among White prostate cancer survivors. Important symptom burden outcomes such as pain, sleep disturbance, and depression have largely been overlooked. However, previous research and our preliminary data suggest that African American prostate cancer survivors likely experience significant symptom burden for outcomes that are generally avoidable and treatable. This project aims to (1) compare symptom burden (i.e., pain, depression, and sleep disturbance) between African American and White prostate cancer survivors, (2) identify modifiable risk factors for worse symptom burden among both African American and White prostate cancer survivors, and (3) identify non-modifiable risk factors for worse symptom burden. This will be the largest and most comprehensive study we are aware of to compare symptom burden between African American and White prostate cancer survivors and to examine genetic predictors of symptom burden in this population. Identifying risk factors for these outcomes will provide (1) critical data regarding symptom burden disparities in African American prostate cancer survivors, a large and growing population; (2) new multilevel modifiable targets for new interventions to reduce disparities in prostate cancer symptom burden; and (3) unique data on multilevel non-modifiable risk factors that will identify which groups are at greatest risk and appropriate for monitoring and early intervention.
