A Self-Administered, Motor-Free, Cognitive Screening Battery for MS: Development and Initial Validation
Abstract
This study addresses Focus Area 3, Biology and Measurement of MS Symptoms. It involves a new approach for screening of cognitive symptoms in adults with MS. In this study, we propose to refine and test a new, self-administered, tablet-based cognitive screening tool we are developing. The Self-Administered Neuropsychological Assessment Battery (SANAB) is a portable, standardized testing platform that assesses multiple cognitive abilities that can be affected by MS. A unique feature of the SANAB is that it uses speech recognition technology. This facilitates assessment of certain aspects of cognition that cannot otherwise be assessed with a computer-based approach. In addition, by speaking their responses, the person does not have to use a mouse or make manual responses, which is advantageous for those who have difficulties with fine motor dexterity or arm movement. Cognitive symptoms affect 40% to 60% of adults with MS. However, in some clinics, patients receive little to no cognitive screening. Cognitive testing in clinical trials is also sometimes cursory. One barrier to access to cognitive screening is the need to have a trained test administrator available to administer the tests. Having a self-administered cognitive screening tool would remove this barrier. Computerized testing such as the SANAB does not replace the neuropsychologist in the clinical evaluation of individuals with MS, but does provide an additional screening tool that, implemented appropriately in the context of a comprehensive care plan, stands to significantly improve the clinical care of people with MS. It takes years for a new cognitive assessment tool to become widely available. However, if the results of this initial study are promising, it will motivate longer-term investment in developing this approach.
