POSITIONED (Precision, Standardization and Organizational Readiness in TBI Care)
Abstract
Unwarranted care variation is differences in patient care that are not explained by differences in patient illnesses or preferences. In other words, patients receive different treatments for reasons not explained by their illnesses. In medicine, unwarranted care variation has been documented for decades and found in nearly every specialty, condition, and procedure in which the topic has been studied. It commonly associated with higher costs, poorer outcomes, or both. The TBIPHRP TRA Treat sub-areas encourage both the standardization (adoption of evidence-based treatments and clinical practice guidelines) and personalization (tailoring of treatment) of care for TBI. Balancing these two goals would eliminate unwarranted care variation. The goal of the proposed Precision, Standardization and Organizational Readiness in TBI Care (POSITIONED) study project is to test a method to reduce unwarranted care variation that has been effective in dozens of other areas of care but has not been tested in TBI rehabilitation. Potentially, all Service Members with TBI could be helped by this research because it promotes alignment with clinical practice guidelines, which include treatments shown to be effective in the care of Service Members with TBI. If we can reduce unwarranted care variation, we reduce the exposure of Service Members to care that will not help and increase their exposure to care that will likely help. The proposed work is directly applicable to clinical care and promises improved care quality in care in 2-3 years. There appear to be many benefits but no tangible risks to Service Members or to the clinical teams that treat them.
